Some Reclaim Your Health services may invite you to provide information about your weight, health history, lifestyle, nutrition, medications, laboratory values, symptoms, or wellness goals.
This information may be used to personalize your educational or coaching experience.
Providing health-related information through the website, metabolic-weight assessment, coaching program, or consultation does not by itself establish a physician-patient relationship or constitute medical care.
The metabolic-weight assessment is an educational profile and is not a validated diagnostic instrument. It does not diagnose prediabetes, diabetes, insulin resistance, metabolic syndrome, or any other medical condition.
